Acclivity Health
Website redesign for a healthcare technology company helping seriously ill patients get the right care, in the right place, at the right time

Overview
Acclivity Health had built something genuinely important: a cloud-based platform that connects hospices, care providers, accountable care organizations, and health systems to help seriously ill patients navigate toward better, more appropriate care. The platform serves more than 4 million patients across all 50 states, and over 70% of high-needs ACO REACH organizations had chosen it as their solution.
Their website needed improvement. Built on a complex Elementor stack, it had a Lighthouse performance score of about 40%, and the messaging was outdated compared to the company’s current direction. For a B2B healthcare technology company serving ACO directors and hospice administrators, who often have limited time and a discerning approach, a site that feels slow and outdated can impact credibility.
"Their attention to the design process left from the get-go has made me very comfortable. I’ve worked with other companies doing similar projects, and they usually haven’t put much importance on the design aspect. Temper And Forge is a very design-focused group that can back up their designs technically, which is rare; usually, agencies aren’t able to do both pieces."

Strategy
The decision to rebuild from scratch in WordPress using Advanced Custom Fields — rather than migrating the existing Elementor setup — was a design decision as much as a technical one. Elementor had made the site slow and rigid; ACF gave the development team a clean, lightweight architecture that could support the visual fidelity the design required without the performance overhead. The performance improvement showed up almost immediately. Diagram was one of the early leaders in this space, with a real track record: over 25 companies launched, including climate-tech unicorns and fintech trailblazers. But their site didn't reflect it. The design was understated, the messaging diluted, and the SEO nonexistent.
Their audience — founders, investors, potential hires — couldn't quickly understand who Diagram was, what they did, or why it mattered.

User experience
Acclivity’s platform targets distinct market segments: Provider Groups, ACO REACH, MSSP participants, payers, and healthcare systems, each with unique clinical contexts and regulatory environments. Navigation was restructured for these audiences, with dedicated paths, clear navigation, and targeted messaging. ACO REACH and MSSP pages reflect their specific regulatory frameworks. The resources section was reorganized for easier navigation, allowing users to find relevant content without sifting through unrelated materials.

Outcome
Within the first week, the site’s Lighthouse performance score jumped from around 40% to 92% — a result that came directly from the architectural decision to rebuild clean rather than optimize on top of a bloated existing stack. The site launched in mid-May 2023, 18 days ahead of the original schedule, after T&F added development capacity specifically to hit the target date.
“Temper And Forge is a very design-focused group that can back up their designs technically, which is rare,” Boborci said. “Usually, agencies aren’t able to do both pieces.” For a company where the message and the platform needed to feel equally credible — where clinical authority and design quality were both on the line — that combination was exactly what the project required.
